Health Care Card Renewal
Is your Health Care Card Expiring?
Check the bottom right hand corner of your Health Care Card for expiry date. You can apply for a new card six months in advance of your Health Care Card expiring. To apply, print and fill the health care card renewal application form here.
To ensure you get your new Health Care Card, please notify the Department of Health and Social Services if within the last 4 years:
- your name (PDF);
- your address (PDF); or
- your aboriginal status (PDF) has changed.
Students who will be studying outside of the NWT for more than three months will need to fill out a temporary absence form (PDF). For more information on health care coverage while attending an educational institution outside the Northwest Territories, please see Studying Outside the NWT.

Note:
You need to fill out a temporary absence form (PDF) if you are outside of the NWT for more than 3 months.
You can use a Change of Address (PDF), Change of Name (PDF) or Change of Aboriginal Status (PDF) form if your personal information has changed.
