Health Care Card Renewal
Your NWT Health Care Cards may expire on March 31, 2010. To ensure that you receive your new Health Care Card, please notify the Department of Health and Social Services if within the last 4 years:
- your name;
- your address; or
- your aboriginal status has changed.
If you have already updated your address (or your address has not changed) within the last 4 years, an application for your new health card will be mailed to you in September 2009.
Students who will be studying outside of the NWT for more than 3 months will need to fill out a temporary absence form. For more information on health care coverage while attending an educational institution outside the Northwest Territories, please see Studying Outside the NWT.
Once the Department receives your current address, an application for your new health care card will be mailed to you.

Note:
You need to fill out a temporary absence form if you are outside of the NWT for more than 3 months.
You can use a Change of Address, Change of Name or Change of Aboriginal Status form if your personal information has changed.
