Population Health Fund (PHF)
The goal of the Population Health Fund (PHF) is to increase community capacity for action on or across determinants of health. Through project funding, the PHF supports initiatives that facilitate coordinated action among voluntary organizations, service providers, governments and the private sector to improve population health.
