Information Request

The Children and Family Services Information Request form must be completed by individuals wanting to access information from their Children and Family Services file.

The completion of this form allows the Director of Children and Family Services to disclose information under the Child and Family Services Act.

The form specifies the type of records to be released, the individual who the records pertain to and to whom the records can be released.

There is no fee required for individuals wishing to have an information request processed.

Process for Accessing Information

  • Download and complete the Children and Family Services Information Request form.
  • Upon receipt of the completed Information Request form, the Department will mail out a letter of acknowledgement to the person who requested the information, and will begin searching for all files relevant to the information request.
  • The Department will coordinate the copying and severing of all records within the file. Legal counsel will complete the severing process. Severing of the records means that all third party references will be removed in order to protect the privacy of those individuals.
  • The Department will send out the package of severed records by registered mail to the individual requesting the information.
